Hypnotherapy Works is providing this notice so that you have information on how I collect and process personal data. Please read this privacy notice as it is important information that you need to know.
What information do I collect?
I collect information from you that is be classed as “Personal Data”, personal data is information on its own or connected with other data that can identify you. The personal Data I hold may include:
- Your full name and contact information (address, email and phone numbers)
- Your calls and correspondence with me
- Your feedback
Personal data will be collected by me.
Why do I need your personal data?
I need personal data from you to be able to work with you in therapy, I will also need personal data to be able to book appointments, for my own records, to contact you about appointments if needed.
I will not ‘trade’ your personal data and I will not sell or rent your details.
Who will your personal data be shared with?
Hypnotherapy Works does not share data with third-party suppliers.
I do have supervision sessions and clients may be discussed during these sessions, however no personal data or any data that may identify the client will be used.
Should the client or any other person be at risk of harm then I have a duty of care to pass this on to the relevant authorities, I would seek the client’s permission where possible before doing this, my duty of care is to the client and others well-being.
I may contact the clients GP should I need to and I use the clients full name, DOB and first line of address.
What rights do you have to amend personal data?
You have the right to review the personal data held by myself and have inaccurate information about you corrected. To request access to your personal information please contact me, email email@example.com
How long do I keep your personal data?
I am required to keep your personal information for 7 years following therapy sessions, if you are under 18 years then it is for 7 years following your 18th birthday.
